To commissioned medical officers, acting assistant surgeons, and others concerned: The following list of diseases and conditions shall be used for the official recording of disabilities after September 30, 1916. No title other than those contained in the following list is to be employed in official returns to the bureau. Rupert Blue, Surgeon General. 3 NOMENCLATURE OF DISEASES AND CONDITIONS. This nomenclature is tentative only and is promulgated, for use until the final report of the Nomenclature Board of the Public Health Service has been received and approved. It consists of the Bellevuo Hospital nomenclature of diseases and conditions, with the terms rearranged alphabetically and numbered serially. Certain changes and additions have been made, as follows: (a) The nomenclature of injuries of the United States Navy has been followed in part for the injuries. (&) The nomenclature of parasites and parasitic diseases was pre- pared by Prof. C. W. Stiles, United States Public Health Service. (c) A few terms have been added to meet the needs of the United States Public Health Service. The following points are to be observed in assigning a diagnosis: (a) Titles in italics are not to be used except when the primary disease is not determined. (&) When two or more morbid entities exist, preference should be given to (I) an infective disease ; (II) the graver affection; and (III) a primary disability over a complication, but care should be taken to state the second condition after the first. Certain titles, as " Abscess, otherwise unclassified," "Fracture, simple, otherwise unclassified," "Gonococcus infection, otherwise unclassified," are intended to cover conditions for which no other title is provided. All gonococcal, syphilitic, and chancroidal infec- tions must be recorded under gonococcal, syphilitic, and chan- croidal titles, and titles such as urethritis acute, cystitis acute, gone- cystitis acute, etc., must not be used as primary diagnoses when due to venereal infection. Infections due to parasites should be reported under the name of the disease, with addition of the site and technical name of the para- site. In case of doubt in regard to the genus and species, the specimen may be sent to the Hygienic Laboratory for determination. When a patient admitted to hospital for a given disease or injury satisfactorily progresses to convalescence from such disease or injury, but, before being sufficiently recovered to be discharged, contracts or develops some other disease not properly a sequel or complica- tion of the original affection or injury, a new bed ticket may be issued by the commanding officer without referring the case to the bureau. In such procedure the number of the original bed ticket 5 6 NOMENCLATURE OF DISEASES AND CONDITIONS. will be referred to and the disease or injury for which the patient was first placed under treatment will be stated, together with the result of the treatment of the same. An approximate or general diagnosis will be accepted as sufficient in eases where it is found impracticable to make a specific diagnosis, but only the titles in this nomenclature are to be used. In all cases where a specific diagnosis is not given it must be supplied afterwards by letter as soon as ascertained, and in every instance where an error has been mado in a diagnosis it is to be corrected immediately by letter. The class numbers in Roman numerals (see classification, p. 7) and the numbers of the international classification of causes of death are given for reference only and not for use in official reports. It is desirable to have injuries classified according to both their nature and their causes. Provision has, therefore, been made for showing the cause of the injury as well as its nature. The classification of causes should be made as follows: A. Suicidal. B. Homicidal. C. Conflagration. Includes all injuries incident to general conflagration. Burns otherwise received are not classed hereunder. D. Accidental drowning or submersion. E. Traumatism by firearms, accidental. To include all injuries caused by the projectile, the blast from great guns, or from the piece when fired. F. Traumatism by explosion. To include powder, gas, compressed air, or steam explosions, also the explosion of a gun. G. Traumatism by fall. H. Traumatism by automobiles. I. Traumatism by aeroplanes. J. Traumatism by railroads. K. Traumatism by other vehicles. L. Traumatism by machines. M. Traumatism by other crushing. N. Traumatism due to athletic sports. 0. Effects of electricity. P. Industrial injuries. Q. Injuries by animals. R. Injuries at birth. S. Casualty in action. T. Traumatism due to violence other than noted above. The above key letters are to be used, for example, as follows: Gunshot wound of brain (A), showing that it was inflicted w T ith suicidal intent; (B), that it was homicidal; while if received in action the entry appears as (S). Accidental drowning or submersion should appear with the suffix (D), while if due to the destruction of a ship by fire as (C); if incident to the explosion of a magazine as (F), unless the same is incident to action, when it should appear as (S). NOMENCLATURE OF DISEASES AND CONDITIONS. 7 In compiling Forms 1923 (Annual Medical and Surgical Report of Out-Patients) and 1924 (Annual Medical and Surgical Report of Hospital Patients) only such diagnoses as have the same title, in the case of diseases, and the same title and key letter, in the case of injuries, are to be combined. For example: "Gunshot wound of brain (A) " is not to be combined with the same title but having the key letter (B). In reporting cases of illness or injury on Forms 8998 (Monthly Report of Patients Admitted to and Discharged from Hospital), 1923 (Annual Medical and Surgical Report of Out-Patients), and 1924 (Annual Medical and Surgical Report of Hospital Patients) the number preceding the title must be given as well as the title. For example: 779. Measles. In compiling Form 1925 (Annual Report of Surgical Operations) the diseases or injuries are to be listed in accordance with the order of the numbers and titles in this list, the description of the opera- tion and other data being placed in the proper columns.
