[12.] The fact that a man or boy, belonging to an Approved Society, has joined His Majesty's Service, does not involve his ceasing membership of that Society, and Bw' ln [13.] n 0n the contrary, hecontinues membership of his Society, except in special cases, such as that of a Society organised solely for a particular firm's employees, when he may be required by its rules to transfer to another 4 society within a specified time after leaving the firm's employment. Men who desire to avail themselves of this right have TWO tiiimis TO DO: — . (1) To apply for membership to the Society they wish to join : then, (2) Immediately, and without waiting to learn from the Society the membership number which it is assigning to them, to report to the Ship's Office the name and address of the Society, together with the name and address ol the agent through whom the application is made. [23.] Membership of the Navy and Army Insurance Fund ceases at once on discharge from His Majesty's Service. 24. 1 .Men who have been members of it up to tlie time of discharge must then — (I) Join Approved Societies within three months after di-charge.rt discharged in Hi. The Statutory Deduction o/iJA a week. 138.] The statutory deduction for National Health Insurance is to be made from the pay of all men and boys who are compulsorily insured or elect insurance, and, t s the period from mid- 16 years of age) is bomo for pay, and in addition for fractional portions of a week, as follows : — (1) For the week in which a man (or boy over 1(5 years of age) is entered or re-entered, unless his Civilian insurance card already bears a National Health Insurance stamp affixed to it in respect of that week. (N.I.I.— 88.) (2) For the week in which a man rejoins lor pay from Prison or Detention Quarters or Civil Power, and also for that in which a man or boy rejoins from D.S.n., unless his pay on being D.S.Q. ceased in (3) For the week in which a boy attains the age of 16. [39.] Care is therefore to be taken that no contri- butions are charged for weeks during the whole of whiob a rating is off pay in Prison, or under Detention, or D.S.Q. in Hospital, but that a contribution is charged lor the frac- tional portion of the week in which he re-enters from Prison, Detention, &c. I 40. t As men undergoing confine- ment in cells do not cease to be borne for pay, insured persons so confined are obargeable with the statutory [41.] The gross amount of the deductions which have become due to date is to be charged on the Ledger or other pay document on the following occasions : — (1) On discharge to another ship, to or from the Coast Guard, from one Coast Guard district to another, and to or from Marine Headquarters ; (2) On being marked "Run" or "Run Qy.," and on being " Discharged Dead " ; 8 (••1) On discharge to the Shove, to Prison, Detention Quarters or Civil Power, and on being " D.S.Q." for pay ; and (4) At the end of every Quarter before the Ledger or other pay document is closed. [42.] In making these charges it is not admissible to ignOre fractions of a penny, but in every case the amount oharged mnsi represent an exact multiple of I '.. [47.] Non-Commissioned Officers, men and boys of the Insurance Number, a-iyned p. the n the l)i\isi,,nal Pay Lists. These numbers are to be quoted in red ink- under the men's names on the Embarkation Muster Rolls, and are to be entered on the Ship's Ledgers in the same in their last Ships would be entered (Article K.<>7 of the King's Regulations). Benefit. [48. J The wife of an insured seaman or marine is on confinement entitled to " Maternity benefit " in respect of her husband's insurance, provided iliat 2C> weeks have elapsed since Ids entry into insurance, and at least '2(1 weekly contributions have been made in respect of him. [49.] The wile of a man, who on re-engagement elects to discontinue his insurance, ceases to be entitled to the benefit from the date of her husband's re-engagement or the date of completion of his first engagement, whichever is the later. ,50.' Similarly the wife of a man who, on promotion, ceases to be insurable under section Hi is not entitled to maternity benefit in respect of a confinement taking place subsequent to his promotion (N.I.I. 35). [51.1 Maternity benefit oan only he paid to the husband himself if the wife dies ..r if he is authorised by her to receive it on her behalf: in the latter case he is required to pay it over to her. [52|.] Wben a marriage oertifioate^is not aheady avail- person having the care of the register in which the marriage is recorded. [53. ! A man who is a member of an Approved Society must arrange direct with his Society for payment of the benefit in accordance with the Society's rules. [54.] If the husband belongs to the Nav> and Ann} Insurance Fund, i.r.. has not joined an Approved Society, the Accountant-Genera] will, on receipt of Form S. 1086 duly completed, arrange lor the benefit to be paid to the wife. (For procedure to be followed, see N.I.I.— 134-145.) [55. 1 If the wife is herself an insured person, she is entitled, subject to any conditions imposed by the rules of her Approved Society, to receive maternity benefit from it in addition to that paid in respect of her husband's Arrears during Service. [56.] If a man falls into arrear with his contributions he runs the risk of reduction or suspension of benefits. [57.] Arrears would ordinarily arise during a man's service in cases of imprisonment, detention and desertion only. Any contributions unpaid during sickness in hospital after pay has stopped will not count as arrears. 10 [58.] A .nan may pay up arrears whie* arise during Lis service l.v affixing stamps at tin- rate of 'M. a week to an arrears card, and he will then be in the same position as if those arrears had not arisen. [59.] A member of an Approved Society can obtain an arrears card from his Society, and a member "I the Navy and Army Insurance Fund from the Secretary, National Health Insurance Commission, 29, Queen Amies Gate, S.W. Discharge, [63.] On discharge from the Service of an insured person (or on his ceasing to be insured on promotion or by S. IH-'U, showing detail- of his i„s„i-a„ee. is given to him, a duplicate of it being forwarded to the Accountant* General. 1 64.] As the certificate takes the place of an send his copy to the Approved Society to which 'he belongs at the time of his discharge or of which he becomes a member after discharge. Form S. 1031. [75.1 Tl.is return is to l>e rendered monthly by all Ships. Establishments, Coast Uuard Districts and Marine Divisions; also by each Ship on paying off. [76.) A " Nil " Return is to he rendered when there are no entries or changes to report. [77.] All relative declarations on Form S. 1082 are to he transmitted as enclosures to the return (N.I.I. — 118), (i) Entries and re-entries with full particulars of their insurance prior to entry or re-entry: the par- ticulars required are explained below (X.I.I. — 86) ; (ii) Re-engaged men ; and (iii) Approved Societies joined within a period .if six months from entry or re-entry. [78.] After the expiry of that period, ratings become members Of the Navy and Army Insurance Fund i N.I.I. require to be- madi the Supplementary return on Form S. 1031. [81.1 Many Societies, which are now "Approved," were in existence as Insurance Companies or Friendly Societies before the National Insurance Act came into operation, and merely formed separate sections to admin- ister the Act. [82.] A man may consequently be a member of such a Society— (1) for private purposes (e.g., for death benefits), or (2) for National Health Insurance purposes, or (3) for both purposes. [90.] The address of the Head Office of an Approved Society is therefore only to be given on Form S. 1031 when the Society is one that does not operate through branches. 191.] A complete list of decentralised Societies is given on pages 81-35. [92.] When the necessary entries have been made on Form S. 1031, the insurance card and book are to be returned to the rating concerned, together with a certificate on Form S. 10:« if he is a member of an Approved Society. (N.I.I.— 28.) [93.] The following additional particulars are to be furnished on Form S. 1031 rendered by Royal Marine Divisions where alterations occur in the period covered by the Return : — (i) All alterations in the Divisions and Divisional numbers of the men including the re-allooatior) of Divisions by the Depot R.M. (ii) On re-enlistment, the man's former Division and Divisional Number, his last Insurance Number, if known, and the date of his discharge, (iii) On recovery from desertion the date of the man's desertion, and his last Insurance Number, if known. Notes. [94,]_(1) The "Approval number" of a Society is the consecutive number under which il lias been approved decentralised one, from the number' of the branch through which membership was effected. It is immaterial whet her approval numbers are reported on Form S. 11)31 and no attempt need be made to ascertain them except in any particular instances in which it is requested that they may be reported. e 22800 B 16 (2) Those branches of a Friendly Society which are located outside the United Kingdom are not Approved Societies under the National Insurance Acts. Form S. 1031 (Supplementary return). [95.] In addition to the ordinary return on Form S. 1031, a supplementary return, devoted exclusively to transfers, is for the present to be rendered^ on the same end 'of each month and on paying off), but there is no objection to an interim return being forwarded whenever it is considered that the number of cases involved makes the adoption of that course desirable. [96.1 The return is to be arranged under two heads as follows:— (i) Transfers from one Approved Society to another such Society. (ii) Transfers from the Navy and Army Insurance Fund to Approved Societies. (N.I.I.-17.) [97.] Each entry under (\) is to show— ( ^ tion to withdraw from the Societv to'which he originally belonged; and (<() Whether he has received that Society's consent to his withdrawal. [98.] In no case can a man or boy be a member of more than one Approved Society for National Health 17 Insurance purposes at the same time, but this does not in any way interfere with his right of joining societies for private purposes. (N.I.I. — 82.) [99.1 When an insured person wishes to transfer from one Approved Society to another the Secretary of the Society /" which he wishes to transfer will, as a rule, supply all information required and make the necessary arrange, ments. [100.1 Entries under (ii) are to consist of reports of the Approved Societies which men have made application to join on transfer from the Navy and Army Insurance Fund. [101.1 It should he observed that a man who joins an Approved Society within the prescribed ti alter entry or re-entry (N.I.I. — 15) will not have become a member of the Navy and Army Insurance Fund ; in such cases, therefore, the report as to the Society joined will require to be made on the ordinary return on Form S. 10:} 1 . (N.I.I.-77 (iii) ). [102.1 As soon as a man notifies the Ship's Office that he has applird to join a Society on transfer, the necessary particulars are t" I ntercd "ii the supplementary return, Admiralty at the time when it is co nmicateil to the man. [104.] Men who apply for membership to a Society, but are subsequently notified that it is not prepared to accept them as members may, on receipt of such notifica- tion, apply to another Society, if they wish to do so. [105.] No entry is to be made on Form S. 1031 (supple- mentary ) in respect of any man who is shown by records on board to be uninsured, but he is to be informed that his election of non-insurance is binding on him. (N.I.I.— 111 ) [106.] He should then notify the Society which he applied to join on transfer from the Navy and Army Insurance Fund that, as he is uninsured and consequently not a member of that Fund, the application was made under a misapprehension. [107. 1 Any case, however, in which the 18 man alleges that he elected to be insured and that he is consequently incorrectly shown as uninsured, is to be referred to the Accountant-General for investigation. [108.] Attention is particularly drawn to the necessity ot reporting not only the full title of the Society but the name and .Ml address of the Agent through whom the •Wlicahon is made. The Accountant Officer is to hand the upper part of Form S. 1030 to the man, who is to forward it direct to the Accountant General. 140.: When posted at home, it will not require to lie stamped, but when posted abroad, it will be subject to the usual rules as to payment of [141.] The lower part of the Form is to be detached by the Accountant Officer and forwarded by him on the same day as t hat on which it is signed by the man to whom tl pper part islanded. ^ 142. In any ease in which a saved if 'full part'ic'ula'rs'a re given at the time of forwarding Form S. 10:30. [143.] On receipt of the two parts of Form S. L086, the Accountant Ueneral will send a form to the wife for her use in intimating when the confinement actually takes place. (7) The foregoing provisions of this section shall, subject to such adaptations, and modifications as may be prescribed, apply to men belonging to the Navul Reserves when employed on service during war or any emergency, and to men of the Army Reserve when called out on permanent service, and to men of the Territorial Force when called out on embodiment, but, except ns aforesaid, shall not apply to any such men. [156.] Whilst undergoing drill or training, or serving voluntarily during manoeuvres, those Officer* and men of the Hoyal Naval Reserve (including Trawler (Section), Hoyal Naval Volunteer Reserve, and Hoyal Fleet Reserve, Imme- diate Class and Classes A and 1$, who were insured persons in civil life immediately before such drill or training, &c, are to be treated for insurance purposes in accordance with the following instructions, which deal both with Part I. of the Act, relating to Health Insurance, and with Part 11., relating to Unemployment Insurance. [157.] These instructions apply also to men of the Royal Naval Auxiliary Sick Herth Reserve when undergoing the Hospital or Man-of-W'ar Course of one week's duration, but not to those whose course consists of 14 attendances of 2 hours each. Method of Paying Contributions. [158.] Contributions under Part I are paid in civil life by means of Health Insurance stamps affixed to cards issued by the National Health Insurance Commission, rs, and those under Part II. by means of Unemployment Insurance stamps affixed to books issued by the Board of Trade. [159.] The same method is used during drill or stamp to be affixed to a Health Insurance card or a Health Insurance stamp to an Unemployment Insurance book. Weekly Rate of Contribntim,. [160.] Contributions are fixed at a weekly rate under both Parts of the Act, but it is important to notice that the week is differently defined for Health and Unemployment Insurance purposes. [161.] Thus for the former the week is the calendar week from midnight on one Sunday to midnight on the following Sunday, while for the latter it is any complete period of seven days from the date on which the employment, i.e., in this case the drill or training, Ac, [162.1 The weekly rate for Health Insurance during drill or training, &c, is 7''. for men employed in civil life in Great Britain and for all members of the Seamen's National Insurance Society, and 5M. for men employed in Ireland who are not members of that Society. [163.] The weekly rate for Unemployment Insurance is 5d., except for men under the age of 18, in who6e case it is 2d. 40 Fractional Port urns of Weeks. [164.] In the case of Health Insurance any fractional portion of a week either l>y itself or over and above a number of full weeks counts as a full week, and any stamp affixed in respect of it must be at the full weekly rate of Id. or SJi. (See aim N.I.I.— 166 and 170. J [165.1 In the case of Unemployment Insurance a frac- tional portion of a week amounting to three days or more, either by itself or over and above a number of seven-day weeks, counts as a full week, and a stamp must be affixed at the full weekly rate of 5d, in respect of it; but only a 2d. stamp is to be affixed in respect of a fractional portion of a week if it does not exceed one day, and a id. stamp if it exceeds one day but does not exceed two days. Number of Stamps required during Drill or Training. \ 166.1 A Health Insurance stamp is to be Affixed to the Health Insurance card of an insured reservist tor every Monday on which he is borne for pay, and in addition for fractional portions of a week in the' same circumstances in which deductions are chargeable against seamen and marines. (N.I. I.— 38, 39, and 170.) 1 167.] In cases in which the card is not already stamped for the week in which a man j s for training, and his card is therefore stamped on board for that week, a notation is to be made on the ledger as follows : — " H. I. S. issued from_ ." 1 168.1 I» all cases in which Irish stamps are issued the word " Irish " is to be written in the N.H.I, column. 1169.] An Unemployment Insurance stamp is to be affixed to an insured reservist's Unemployment Insurance book for every complete period of seven days during which he is borne for pay, irrespective of any stamps already affixed on accounl of civil employment, and in addition for any fractional portion of a week between the termination of the last full week and the date of discharge. (N.I.I.— 165.) Illustrations of Numbers of Stamps required. 1170.] The following examples are given to illustrate the until,. dot calculating the number of stamps required — (1) If a man insured in Great Britain joins on a Monday and serves for 28 days, four Id. stamps should be affixed to his Health Insurance card and four U. stamps to his Unemployment Insurance book. 41 (2) If he joins on a Thursday and serves for .iO days, four 7rf. stamps should he affixed to his Health Insurance card if it already hears a stamp lor the week in which the Thursday falls, and live if it does not already hear a stamp for that week. (In the latter case the notation "1I.I.S. issued from_ " will appear on the ledger.) Four bd. stamps and one Ul. should he affixed to his Unemployment Insurance hook. (3) If a man insured in Ireland joins on a Monday and serves 17 days, three 5Ji. stamps should he affixed to his Health Insurance card and three 5d. stamps to his Unemployment Insurance book.
