Department .of the Navy, Bureau of Medicine and Surgery, January 1. 1919. GENERAL INSTRUCTIONS FOR BLANK FORMS (PERSONNEL). Ribbon copies (originals; only of reports and returns shall be forwarded to the department, except 'where a form is required in duplicate or more copies, in which case clear carbon" copies are permitted as the duplicate, or seconds, thirds, etc. 2. No form shall be extended by attaching pieces. A whole additional sheet of a form shall be used when necessary, or additional pieces pasted or fastened in such a manner so as not to increase the original size of the form. '.'<. Economy in spacing shall be exercised, and when not specially ruled, single- line spacing with the typewriter should always be used. Do not write or place other markings in spaces when no information is required. Leave them blank. 4. All reports and returns shall be forwarded at the time required, whether there ie anything to report or not. Receipt of a form with no information at least informs iIih bureau that it has not been overlooked, or that it has not been lost in the mails. 5. All cases remaining on the sick list on December 31 shall be reported as "Con- tinued to next year" on Form F cards, the appropriate entries made on Forms F and K and new cards and entries made for the ensuing year. Tv:o Form F cards are required for each case continued to next year. On the Form F card for the year just ended a shall be entered under •"disposition."' with the number of sick days from date of taking up to December 31. On the Form F card for the ensuing year a shall be entered under "Taken up as," followed by the date taken up in the previous year, but when finally disposed of, only the sick days for the current calendar year shall be reported on this card. 6 . Patients transferred to other than naval institutions should not have then records closed, except as provided for in the manual. In the case of treatment of persons on leave from their regular ship or station the medical officer at the place to which the person is attached, as "tfell as- the bureau, shall be informed. 7. Names. — Blank forms, letters, and telegrams, generally, which refer to person- nel, deal primarily with the individual, and, as most all forms and correspondence p. re filed in jackets, under the name, it is imperative that the Christian name or names s-nd surname be spelled out in full as far as possible. 4 NOMENCLATURE OE DISEASES AND INJURIES. Owing to the frequency with which some names appear in the files, it is at times impossible to differentiate between individuals whose names are similar when only the surname and initials are given. 8. Grades and rates. — As far as possible, the grades and rates of officers and enlisted men shall be spelled out in full, but where sufficient space is not provided, the fol- lowing abbreviations shall be used: OFFICERS. (9) Nomenclature instructions. — (a) The following lists of diseases and injuries shall be used for the official recording of disabilities and in all forms and returns to the department. These lists are composed of titles accepted by the commission on clinical records of the Bellevue Hospital, New York City, with some changes deemed desirable for naval use. NOMENCLATURE OF DISEASES AND INJURIES. 7 (b) The class numbers are given for reference only, not for use. (c) Titles in italics, which are symptoms, are not to be used except when the pri- mary disability is not determined. (d) When two or more morbid entities exist, preference shall be given to (I) an infective disease; (II) the graver affection; and (III) a primary disability over a complication. (e) Certain titles followed by the word "unqualified" are intended to cover such cases of those disabilities for which no other title is provided, and where occurring the word "unqualified" shall be substituted by the site of the disability or the prin- cipal organ affected, as abscess of neck, etc., or for instance epididymitis or orchitis, when due to a gonococcus infection should be returned as Gon. inf. of epididymis and Gon. inf. of testicle. (/) All gonococcal, syphilitic, and chancroidal infections must be recorded under these titles, and titles such as cystitis acute, epididymitis acute, gonecystitis acute, orchitis acute, urethritis acute, etc., must not be used as primary diagnoses when due to venereal infection. (g) It frequently happens that conditions existing prior to enlistment or appoint- ment are taken up as admissions when as a matter of fact it is a sequel to the condition that should be actually reported, e. g., old wounds are reported that were inflicted prior to enlistment or appointment, as wound, gunshot, etc., when the case should be taken up as a cicatricial contraction, etc. It is a great mistake to charge our Navy statistics with numerous conditions, especially injuries, that were never incurred in the service when the real condition is never reported on the returns. (h) The proper use of the titles "Malingering, " "No disease, " and "Diagnosis un- determined, " is as follows: The first is when a patient claims to be ill or unduly exaggerates a disability when the medical officer is of the opinion that there is none, or only slight actual disability ; "No disease " is to be used for individuals who for any reason have to be carried on the medical department returns, as for rations or as " sus- pects " under observation for infectious diseases, do not claim to be sick and are not regarded as such by the medical officer; and "Diagnosis undetermined" is to be used when there is an evident disability present, the nature of which is, at the time, not evident. The origin of the disability in cases taken up as "No disease" shall be determined in the same manner as prescribed in paragraph 2254, Manual for the Medical Depart- ment, for cases of bona fide disability. The Bureau suggests in connection with the nomenclature title "No disease", that the condition held under observation for, be recorded as follows: "No disease (scarlet fever contact). " (i) Diagnosis undetermined: In order to provide for the admission to the sick list and transfer of patients where, owing to insufficient time or lack of clinical apparatus, it is impracticable for the medical officer to make other than a tentative diagnosis, the use of the designation "Diagnosis undetermined" is made. 8 NOMENCLATURE OF DISEASES AND INJURIES. The object i3 to prevent the recording of diagnoses which are subsequently proved to be erroneous and which at times operate detrimentally to the future professional prospects of the patient. Cases of "Diagnosis undetermined" shall be originally entered as admissions on all medical department returns, the tentative diagnosis, if desired, being inclosed in brackets as "Diagnosis undetermined (measles), " the further notations being made in a similar manner as when a diagnosis is made. The returns of the place to which transferred shall take up the case as a readmission "R. A., " and when the diagnosis is established the case shall be discharged for change of diagnosis "C," and readmitted "R. A.," with the proper diagnosis and other necessary entries made. For the average case three Form F cards are required — 1. Ship (or other place), one card. A-T, Diagnosis undetermined. 2. Hospital (or other place), two cards required. RA-C, Diagnosis undetermined. 3. RA-and final disposition, with title of the diagnosis established. "Diagnosis undetermined" shall only be used where it is necessary to admit a case and transfer it to some other place. Under no circumstances shall a case of "Diagnosis undetermined" be disposed of as to duty, died, invalided from the service, or to sick leave. Cases taken up with "Diagnosis undetermined" and disposed of as "Ran" or "Continued to next year" will, however, have to be reported to the bureau with "Diagnosis undetermined" on the records. (j) Specialty key letters : The specialization of certain classes of naval work and the peculiar duties surrounding the nature of the work makes it desirable to inaugurate a system of special designations for diseases and injuries, and for this purpose the fol- lowing key letters shall be used the same as key letters are now used for injuries: R=Aeronautic duty. For diseases or injuries incident to actual flying. S= Submarine duty. For diseases or injuries incident to actual maneuvering of or accident to the vessel. U= Divers duty. For diseases or injuries incident to actual diving. When used in connection with injury key letters, first give key letter for injury and follow by specialty key letter, e. g., drowning incident to flying, (D-R); when in submarine, (D-S); or while diving, (D-U). The specialty key letter shall not be used for all persons attached to special duty, but only in cases of actual specialty work, that is, a person attached to aeronautic or submarine duty who is disabled on ordinary occasions should not have a specialty letter. (k) No other titles than those given shall be used as prefixes for injuries and shall be followed, as required, by the site of the principal traumatism or organ injured and the causative agent. For wounds, the character of the traumatism (e. g., incised, lacerated, punctured) shall also be indicated. NOMENCLATURE OF DISEASES AND INJURIES. 9 (I) Under poisons, follow the word poison by the name of the substance, and where more than one condition of poisoning is recognized state either acute or chronic. (m) For the titles of all injuries (wounds, etc., and poisons) a key letter referring to the cause under which the injury was received is required on all forms. In the health record and on forms not specially ruled the key letter is to follow the title, as "Wound, lacerated, neck, shrapnel, (K), " etc. (n) Inasmuch as a correct translation of naval statistics of injuries into those of the International Classification requires an arrangement according to causes, it is neces- sary that the returns show the cause as well as the nature of the injury and causative agent. The classification of causes desired by the Bureau is as follows: A. Suicidal. B. Homicidal. C. Conflagration. Includes all injuries incident to general conflagration. Burns otherwise received are not classed hereunder. D. Accidental drowning or submersion. E. Traumatism by firearms, accidental. To include all injuries caused by the projectile, the blast from great guns, or from the piece when fired. F. Traumatism by explosion. To include powder, gas, compressed air, or Bteam explosions, also the explosion of a gun. G. Traumatism by fall. H. Traumatism by machines. I. Traumatism by other crushing. J. Traumatism due to athletic sports. K. Casualty in action against organized enemy. L. Traumatism due to violence other than noted above. The above key letters are to be used for example, as follows: Wound, punctured, of brain, pistol bullet (A), showing that it was inflicted with suicidal intent; (B) that it was homicidal, while if received in action the entry appears as (K). Accidental drowning or submersion should appear with the suffix (D), while if due to the destruc- tion of a ship by fire as (C); if incident to the explosion of a magazine as (F), unless the same is incident, to action, when it should appear as (K).
