When the disease for which the patient applies for treatment i^ dependent upon another coexisting disease, the latter should be named as the principal disease and the former as the complication— e. g., valvular disease, chronic cardiac, and dilatation, chronic cardiac; tuberculosis, chronic pulmonary, and pleurisy, serofibrinous; diabetes tnellitus and gangrene of toes. The international [lumbers are given for reference only. The Roman numerals in the second column of the left margin classify the titles as to their reliability as statements of cause of death. The seven classes referred to are as follows: Class I. Reliable statement of cause of death without autopsy. Class 11. Unreliable statement of cause of death unless verified by autopsy. (lass III. Unreliable statement of cause of death uniess verified by autopsy or operation. Ciass IV. Unreliable statement of cause of death unless verified by autopsy or confirmatory laboratory tests. Class V. Unreliable statement of cause of death unless verified by autopsy or by finding of etiological organism. Cla^s VI. Unreliable statement of cause of death unless supported by a statement of conditions adequate to cause death as contribu- tory cause of death or a statement showing the process through which the death occurred. Class VII. Inadequate statement of cause of death and not to be used as statement for primary cause of death. The application of this classification to the medical certificates of death shall be made in accordance with the ordinance of September 11, 1918, "An ordinance to better provide for the recording of vital statistics and for other purposes."
