 1. In compliance with your instructions a nomenclature for dis- 
 eases and conditions has been complied for the department of health 
 and is herewith submitted. 
 
 2. The terms of this nomenclature have been obtained from the 
 Bellevue Hospital Nomenclature of Diseases and Conditions. Sonic 
 modifications of this nomenclature found in the Nomenclature of 
 Diseases and Injuries. Medical Department, United States Navy, 
 have been adopted. 
 
 The phraseology of some of the titles of the Bellevue ' Hospital 
 nomenclature has been changed and many titles have been suffixed 
 by a " Desirable qualification" (D. Q.) which corresponds to the 
 "Secondary diagnosis" requested in the Bellevue Hospital nomen- 
 clature under many of the titles. This has been done in an effort 
 to use a minimum of explanatory remarks without inviting ambi- 
 guity. For the same purpose the method of printing the titles, which 
 are only symptoms, in italics used in the Navy nomenclature has 
 been followed. • 
 
 3. The diseases of the mind have been classified and named in 
 accordance with the system described by William A. White, M. D.. 
 and Francis M. Barnes, jr., M. D., in a paper, "A plan for indexing 
 cases in hospitals for the insane" (Am. Jour. Insan., Vol. LXIX. 
 No. 1). It was thought necessary to include these authors' explana- 
 tory notes in order to make it possible for the physician to apply the 
 titles to the conditions for which they were intended. 
 
 4. The new feature of the nomenclature, the classification of its 
 titles as to their reliability as statements of causes of death, has been

 adopted in order to obtain, primarily and in every case, a statement 
 of cause of death satisfactory to the registrars of vital statistics. 
 This arrangement was suggested by the report on the "Accuracy of 
 
 Certified Causes of Death" by a commit tee appointed by the American 
 Public Health Association (Public Health Reports, vol. 32, No. 30). 
 The opinion expressed by this committee has been the guide in the 
 grading of the terms. 
 
 5. The titles have been grouped alphabet icallv under the organs or 
 structures to which they apply or under other appropriate headings 
 thoughl besl to facilitate the finding of the terms. 
 
 6. An alphabetic arrangement of all the titles independently of 
 these groups also appears desirable. A practical application of the 
 nomenclature would show whether such a need exists. Such test 
 would also help reveal defects and insufficiencies and would be the 
 rational method to increase our efforts for the perfection of this 
 nomenclature. 
 
 7. Instructions regarding the use of the nomenclature and the 
 recording and filing of medical histories are prefixed.

REGULATIONS FOR THE RECORDING AND FILING OF MEDICAL 
 
 HISTORIES. 
 
 The purpose of recording and filing of medical history is twofold : 
 
 1. To place on record the events in the life of the inhabitants of 
 these islands that bring them in contact with the services of the 
 department of health and that go to form the health record or 
 medical history of an individual. 
 
 2. To accomplish records of diseases, injuries, and other medical 
 conditions in the islands which will readily lend themselves to 
 analysis and constitute an authentic source for accurate and scientific 
 clinical, epidemiological, and medicosociological studies and statistics. 
 
 It shall be the duty of the chief municipal physicians to institute 
 such services, procedures, and forms as will accomplish this purpose, 
 in accordance with the rules hereinafter stated. 
 
 The completed medical history of an individual shall contain: 
 
 1. Clinical record of birth and the first few weeks of infancy, to be 
 kept by the attending physician or midwife. 
 
 2. Copy of birth certificate. 
 
 3. Record of infancy and childhood, obtained through infant- 
 welfare work, visiting nurses, and through examinations of school 
 children, etc. 
 
 4. Records of vaccinations and inoculations against infectious 
 diseases. 
 
 5. Records of parturitions and abortions. 
 
 6. Record of clinical and laboratory findings, treatment and result 
 obtained of all medical and surgical diseases, injuries, and conditions 
 for which the individual requests examination, advice, or treatment 
 from physicians of these municipalities or which come under observa- 
 tion of such physicians. 
 
 7. Record of post-mortem examination. 
 
 8. Copy of death certificate.

NOMENCLATURE OF DISEASES AND CONDITIONS. 
 
 These records shall be filed and preserved at the municipal 
 
 hospitals. 
 
 The various records of a medical history shall always be found 
 securely fastened together, arranged chronologically as far as possible. 
 They shall be filed in two series: 
 
 1. Current histories. 
 
 2. Closed histories. 
 
 The current histories (the medical histories of the living popula- 
 tion) shall be filed alphabetically by the surname of the individual 
 and in convenient series according to the year of birth; except the 
 medical histories of persons with records from the current calendar 
 year which histories shall be filed in a separate series until the data 
 for the annual report have been obtained. 
 
 The closed histories (the medical histories of the deceased and of 
 persons past 80 years of age and not known to be living) shall be 
 filed by the disease which, according to the certificate of death, was 
 the primary cause of death: and if no record of death was obtained, 
 according to the principal disease or condition for which medical 
 services had been rendered; provided, however, that closed histories 
 in the file for the current calendar year shall not be removed there- 
 from until the data for the annual report have been obtained. 
 
 No original medical history or other original record shall he 
 removed from the municipal hospitals under any circumstances 
 except upon the order of a court, but copies of such histories or 
 other records may be obtained at the discretion of the chief municipal 
 physicians. 
 
 The medical records obtained of patients isolated in the insane 
 asylum and Leper asylum, St. Croix, may be preserved and filed at 
 these institutions, provided that on the death or discharge of such 
 patients a copy of the medical record obtained be furnished the 
 municipal hospital in which the patient's primary medical history is 
 on file. 
 
INSTRUCTIONS FOR THE USE OF THE NOMENCLATURE. 
 
 The word- and phrases in bold faced type — e. g., ''Peritonitis, 
 acute local, puerperium" arc the exact terms in which the diagnoses 
 
 of diseases and injuries shall be stated on all forms of the department 
 of health and in all reports, and in official correspondence within the 
 department. No other words must be used on the forms in the 
 spaces designated for such diagnoses. 
 
 To the diagnoses of injuries the proper international number 
 denoting the character of the violence (see under Injuries, page LIT;. 
 shall be suffixed within parenthesis — e. g.. "avulsion, complete, of 
 external ear (17oD)". 
 
 Titles with words or phrases under the line must always be com- 
 pleted in accordance with the instructions contained in these expres- 
 sions — e. g,. •alcoholic psychosis, acute alcoholic hallucinosis." 
 
 "Desirable qualifications" (D. Q. » provided for in some title- 
 must be given whenever possible. They shall be written within 
 parenthesis and whenever words or phrases of the qualification are 
 given such words or phrases only must be used — e. g. ; "syphilis of 
 brain with psychosis (Pseudo-paretic). " 
 
 Titles in italics which are names of symptoms only are not to be 
 used except when the primary disease or injury cannot be determined 
 
 All diseases due to venereal infection — gonococcal, syphilitic, and 
 chancroidal — must be recorded under these titles — e. g.. not "sal- 
 pingitis, acute." when due to gonococcal infection, but "gonococcu> 
 infection of Fallopian tube:" not "periostitis, chronic of ulna' 
 when due to s^hilis, but "syphilis of ulna:" not "lymphadenitis. 
 acute of inguinal lymphnodes." when due to chancroidal infection, 
 but ''lymphadenitis, acute of inguinal lymphnodes. chancroidal." 
 
 In stating the relation between the principal disease and the 
 complicating or concurrent disease the following rules should be 
 followed : 
 
 A. Of two independent affections the choice should be made in 
 favor of: 
 
 First. An infectious disease — e. g.. chicken pox and valvular 
 
 disease, chronic cardiac.

 Second. The ,graver affection e. g., carcinoma of uterus and 
 calculus iu bladder. 
 
 Third. The disease for which the patient applies for treatment 
 e. g., acne and arthritis, chronic of knee joint. 
 
 B. When the disease for which the patient applies for treatment 
 i^ dependent upon another coexisting disease, the latter should be 
 named as the principal disease and the former as the complication— 
 e. g., valvular disease, chronic cardiac, and dilatation, chronic cardiac; 
 tuberculosis, chronic pulmonary, and pleurisy, serofibrinous; diabetes 
 tnellitus and gangrene of toes. 
 
 The international [lumbers are given for reference only. 
 
 The Roman numerals in the second column of the left margin 
 classify the titles as to their reliability as statements of cause of 
 death. The seven classes referred to are as follows: 
 
 Class I. Reliable statement of cause of death without autopsy. 
 
 Class 11. Unreliable statement of cause of death unless verified by 
 autopsy. 
 
 (lass III. Unreliable statement of cause of death uniess verified 
 by autopsy or operation. 
 
 Ciass IV. Unreliable statement of cause of death unless verified 
 by autopsy or confirmatory laboratory tests. 
 
 Class V. Unreliable statement of cause of death unless verified 
 by autopsy or by finding of etiological organism. 
 
 Cla^s VI. Unreliable statement of cause of death unless supported 
 by a statement of conditions adequate to cause death as contribu- 
 tory cause of death or a statement showing the process through 
 which the death occurred. 
 
 Class VII. Inadequate statement of cause of death and not to be 
 used as statement for primary cause of death. 
 
 The application of this classification to the medical certificates of 
 death shall be made in accordance with the ordinance of September 
 11, 1918, "An ordinance to better provide for the recording of vital 
 statistics and for other purposes."

 The statement of the nature of the injury must 
 be supplemented by an indication of the character 
 of the violence. The physician shall therefore 
 assign the International number to correspond 
 with the following classification of causes, except 
 where a number has already been assigned. The 
 number shall be written within parenthesis after 
 the diagnosis, e. g., "Fracture of femur (160)". 
 The International numbers are given without 
 reference to possible judicial determination as to 
 the causes of violence, namely, whether accidental, 
 suicidal, or homicidal; therefore the International 
 numbers 153. and 155 to 163, and 182 to 184 are 
 omitted.
